This is more or less added as of commit ac4de70 (2013-01-02) (v 1.13).
Since that commit, the aspect ratio setting "[4] Entire screen, anti-aliased" shows the entire screen (NTSC 256x224) in 256x192, squishing the screen vertically and using 2-pixel averaging to smooth the resizing.
I have a feeling that showing 256x224 in 219.42x192 using BassAceGold's smoothed-resize code and 8-pixel averaging would need far too much processing power.
