Thanks, that's a genuine bug. A skip() or todo_skip() before a plan wouldn't output the TAP header.

A skip() or todo_skip() would not start the stream

If called before a plan is set, there would be no TAP header.

* Most instances of using an object in a boolean context were resolved
  by simply checking if the containing variable was defined.

* new test for objects based on arrays
