Fix broken hyperlinks GitHub handles links of the form [foo](url) incorrectly if there's a space between the square and round brackets. This commit removes those spaces. Actual command: sed -i -e 's/] (/](/g' *.md
Lots of changes to fix broken links due to nasty Github update that made lots of them become invalid.
