[ Upstream commit d754941225a7dbc61f6dd2173fa9498049f9a7ee ] If, for any reason, userland shuts down iscsi transport interfaces before proper logouts - like when logging in to LUNs manually, without logging out on server shutdown, or when automated scripts can't umount/logout from logged LUNs - kernel will hang forever on its sd_sync_cache() logic, after issuing the SYNCHRONIZE_CACHE cmd to all still existent paths. PID: 1 TASK: ffff8801a69b8000 CPU: 1 COMMAND: "systemd-shutdow" #0 [ffff8801a69c3a30] __schedule at ffffffff8183e9ee #1 [ffff8801a69c3a80] schedule at ffffffff8183f0d5 #2 [ffff8801a69c3a98] schedule_timeout at ffffffff81842199 #3 [ffff8801a69c3b40] io_schedule_timeout at ffffffff8183e604 Valera1978#4 [ffff8801a69c3b70] wait_for_completion_io_timeout at ffffffff8183fc6c Valera1978#5 [ffff8801a69c3bd0] blk_execute_rq at ffffffff813cfe10 Valera1978#6 [ffff8801a69c3c88] scsi_execute at ffffffff815c3fc7 Valera1978#7 [ffff8801a69c3cc8] scsi_execute_req_flags at ffffffff815c60fe Valera1978#8 [ffff8801a69c3d30] sd_sync_cache at ffffffff815d37d7 Valera1978#9 [ffff8801a69c3da8] sd_shutdown at ffffffff815d3c3c This happens because iscsi_eh_cmd_timed_out(), the transport layer timeout helper, would tell the queue timeout function (scsi_times_out) to reset the request timer over and over, until the session state is back to logged in state. Unfortunately, during server shutdown, this might never happen again. Other option would be "not to handle" the issue in the transport layer. That would trigger the error handler logic, which would also need the session state to be logged in again. Best option, for such case, is to tell upper layers that the command was handled during the transport layer error handler helper, marking it as DID_NO_CONNECT, which will allow completion and inform about the problem. After the session was marked as ISCSI_STATE_FAILED, due to the first timeout during the server shutdown phase, all subsequent cmds will fail to be queued, allowing upper logic to fail faster. The call to check_disk_change() then saw an event flag indicating a possible media change and tried to flush any cached data for the device. As part of the flush, it tried to acquire the super_block->s_umount lock associated with the cdrom device. This was the same super_block as created and locked by the previous task. The first task acquires the s_umount lock and then the sr_mutex_lock; the second task acquires the sr_mutex_lock and then the s_umount lock. This patch fixes the issue by moving check_disk_change() out of cdrom_open() and let the caller take care of it. [ Upstream commit 934140ab028713a61de8bca58c05332416d037d1 ] cachefiles_read_waiter() has the right to access a 'monitor' object by virtue of being called under the waitqueue lock for one of the pages in its purview. However, it has no ref on that monitor object or on the associated operation. What it is allowed to do is to move the monitor object to the operation's to_do list, but once it drops the work_lock, it's actually no longer permitted to access that object. However, it is trying to enqueue the retrieval operation for processing - but it can only do this via a pointer in the monitor object, something it shouldn't be doing. If it doesn't enqueue the operation, the operation may not get processed. If the order is flipped so that the enqueue is first, then it's possible for the work processor to look at the to_do list before the monitor is enqueued upon it. Fix this by getting a ref on the operation so that we can trust that it will still be there once we've added the monitor to the to_do list and dropped the work_lock. The op can then be enqueued after the lock is dropped. The bug can manifest in one of a couple of ways. The first manifestation looks like: FS-Cache: FS-Cache: Assertion failed FS-Cache: 6 == 5 is false ------------[ cut here ]------------ kernel BUG at fs/fscache/operation.c:494! RIP: 0010:fscache_put_operation+0x1e3/0x1f0 ... fscache_op_work_func+0x26/0x50 process_one_work+0x131/0x290 worker_thread+0x45/0x360 kthread+0xf8/0x130 ? create_worker+0x190/0x190 ? kthread_cancel_work_sync+0x10/0x10 ret_from_fork+0x1f/0x30 This is due to the operation being in the DEAD state (6) rather than INITIALISED, COMPLETE or CANCELLED (5) because it's already passed through fscache_put_operation(). The bug can also manifest like the following: kernel BUG at fs/fscache/operation.c:69! ... [exception RIP: fscache_enqueue_operation+246] ... [ Upstream commit c5a94f434c82529afda290df3235e4d85873c5b4 ] It was observed that a process blocked indefintely in __fscache_read_or_alloc_page(), waiting for FSCACHE_COOKIE_LOOKING_UP to be cleared via fscache_wait_for_deferred_lookup(). At this time, ->backing_objects was empty, which would normaly prevent __fscache_read_or_alloc_page() from getting to the point of waiting. This implies that ->backing_objects was cleared *after* __fscache_read_or_alloc_page was was entered. When an object is "killed" and then "dropped", FSCACHE_COOKIE_LOOKING_UP is cleared in fscache_lookup_failure(), then KILL_OBJECT and DROP_OBJECT are "called" and only in DROP_OBJECT is ->backing_objects cleared. This leaves a window where something else can set FSCACHE_COOKIE_LOOKING_UP and __fscache_read_or_alloc_page() can start waiting, before ->backing_objects is cleared There is some uncertainty in this analysis, but it seems to be fit the observations. Adding the wake in this patch will be handled correctly by __fscache_read_or_alloc_page(), as it checks if ->backing_objects is empty again, after waiting. Customer which reported the hang, also report that the hang cannot be reproduced with this fix. The backtrace for the blocked process looked like: PID: 29360 TASK: ffff881ff2ac0f80 CPU: 3 COMMAND: "zsh" #0 [ffff881ff43efbf8] schedule at ffffffff815e56f1 #1 [ffff881ff43efc58] bit_wait at ffffffff815e64ed #2 [ffff881ff43efc68] __wait_on_bit at ffffffff815e61b8 #3 [ffff881ff43efca0] out_of_line_wait_on_bit at ffffffff815e625e Valera1978#4 [ffff881ff43efd08] fscache_wait_for_deferred_lookup at ffffffffa04f2e8f [fscache] Valera1978#5 [ffff881ff43efd18] __fscache_read_or_alloc_page at ffffffffa04f2ffe [fscache] Valera1978#6 [ffff881ff43efd58] __nfs_readpage_from_fscache at ffffffffa0679668 [nfs] Valera1978#7 [ffff881ff43efd78] nfs_readpage at ffffffffa067092b [nfs] Valera1978#8 [ffff881ff43efda0] generic_file_read_iter at ffffffff81187a73 Valera1978#9 [ffff881ff43efe50] nfs_file_read at ffffffffa066544b [nfs] Valera1978#10 [ffff881ff43efe70] __vfs_read at ffffffff811fc756 #11 [ffff881ff43efee8] vfs_read at ffffffff811fccfa #12 [ffff881ff43eff18] sys_read at ffffffff811fda62 #13 [ffff881ff43eff50] entry_SYSCALL_64_fastpath at ffffffff815e986e Signed-off-by: NeilBrown <neilb@suse.com> Signed-off-by: David Howells <dhowells@redhat.com> Signed-off-by: Sasha Levin <sashal@kernel.org>

[ Upstream commit 42ffb0bf584ae5b6b38f72259af1e0ee417ac77f ] There exists a deadlock with range_cyclic that has existed forever. If we loop around with a bio already built we could deadlock with a writer who has the page locked that we're attempting to write but is waiting on a page in our bio to be written out. The task traces are as follows PID: 1329874 TASK: ffff889ebcdf3800 CPU: 33 COMMAND: "kworker/u113:5" #0 [ffffc900297bb658] __schedule at ffffffff81a4c33f #1 [ffffc900297bb6e0] schedule at ffffffff81a4c6e3 #2 [ffffc900297bb6f8] io_schedule at ffffffff81a4ca42 #3 [ffffc900297bb708] __lock_page at ffffffff811f145b Valera1978#4 [ffffc900297bb798] __process_pages_contig at ffffffff814bc502 Valera1978#5 [ffffc900297bb8c8] lock_delalloc_pages at ffffffff814bc684 Valera1978#6 [ffffc900297bb900] find_lock_delalloc_range at ffffffff814be9ff Valera1978#7 [ffffc900297bb9a0] writepage_delalloc at ffffffff814bebd0 Valera1978#8 [ffffc900297bba18] __extent_writepage at ffffffff814bfbf2 Valera1978#9 [ffffc900297bba98] extent_write_cache_pages at ffffffff814bffbd PID: 2167901 TASK: ffff889dc6a59c00 CPU: 14 COMMAND: "aio-dio-invalid" #0 [ffffc9003b50bb18] __schedule at ffffffff81a4c33f #1 [ffffc9003b50bba0] schedule at ffffffff81a4c6e3 #2 [ffffc9003b50bbb8] io_schedule at ffffffff81a4ca42 #3 [ffffc9003b50bbc8] wait_on_page_bit at ffffffff811f24d6 Valera1978#4 [ffffc9003b50bc60] prepare_pages at ffffffff814b05a7 Valera1978#5 [ffffc9003b50bcd8] btrfs_buffered_write at ffffffff814b1359 Valera1978#6 [ffffc9003b50bdb0] btrfs_file_write_iter at ffffffff814b5933 Valera1978#7 [ffffc9003b50be38] new_sync_write at ffffffff8128f6a8 Valera1978#8 [ffffc9003b50bec8] vfs_write at ffffffff81292b9d Valera1978#9 [ffffc9003b50bf00] ksys_pwrite64 at ffffffff81293032 I used drgn to find the respective pages we were stuck on page_entry.page 0xffffea00fbfc7500 index 8148 bit 15 pid 2167901 page_entry.page 0xffffea00f9bb7400 index 7680 bit 0 pid 1329874 As you can see the kworker is waiting for bit 0 (PG_locked) on index 7680, and aio-dio-invalid is waiting for bit 15 (PG_writeback) on index 8148. aio-dio-invalid has 7680, and the kworker epd looks like the following crash> struct extent_page_data ffffc900297bbbb0 struct extent_page_data { bio = 0xffff889f747ed830, tree = 0xffff889eed6ba448, extent_locked = 0, sync_io = 0 } Probably worth mentioning as well that it waits for writeback of the page to complete while holding a lock on it (at prepare_pages()). Using drgn I walked the bio pages looking for page 0xffffea00fbfc7500 which is the one we're waiting for writeback on bio = Object(prog, 'struct bio', address=0xffff889f747ed830) for i in range(0, bio.bi_vcnt.value_()): bv = bio.bi_io_vec[i] if bv.bv_page.value_() == 0xffffea00fbfc7500: print("FOUND IT") which validated what I suspected. The fix for this is simple, flush the epd before we loop back around to the beginning of the file during writeout. If we failed during initialization of sysfs, we weren't unregistering the top level btrfs sysfs entry nor the debugfs stuff. Not unregistering the top level sysfs entry makes future attempts to reload the btrfs module impossible and the following is reported in dmesg: [ 2246.451296] WARNING: CPU: 3 PID: 10999 at fs/sysfs/dir.c:486 sysfs_warn_dup+0x91/0xb0() [ 2246.451298] sysfs: cannot create duplicate filename '/fs/btrfs' [ 2246.451298] Modules linked in: btrfs(+) raid6_pq xor bnep rfcomm bluetooth binfmt_misc nfsd auth_rpcgss oid_registry nfs_acl nfs lockd fscache sunrpc parport_pc parport psmouse serio_raw pcspkr evbug i2c_piix4 e1000 floppy [last unloaded: btrfs] [ 2246.451310] CPU: 3 PID: 10999 Comm: modprobe Tainted: G W 3.13.0-fdm-btrfs-next-24+ Valera1978#7 [ 2246.451311] Hardware name: Bochs Bochs, BIOS Bochs 01/01/2011 [ 2246.451312] 0000000000000009 ffff8800d353fa08 ffffffff816f1da6 0000000000000410 [ 2246.451314] ffff8800d353fa58 ffff8800d353fa48 ffffffff8104a32c ffff88020821a290 [ 2246.451316] ffff88020821a290 ffff88020821a290 ffff8802148f0000 ffff8800d353fb80 [ 2246.451318] Call Trace: [ 2246.451322] [<ffffffff816f1da6>] dump_stack+0x4e/0x68 [ 2246.451324] [<ffffffff8104a32c>] warn_slowpath_common+0x8c/0xc0 [ 2246.451325] [<ffffffff8104a416>] warn_slowpath_fmt+0x46/0x50 [ 2246.451328] [<ffffffff81367dc5>] ? strlcat+0x65/0x90 (....) [ Upstream commit c5c97cadd7ed13381cb6b4bef5c841a66938d350 ] The ubsan reported the following error. It was because sample's raw data missed u32 padding at the end. So it broke the alignment of the array after it. The raw data contains an u32 size prefix so the data size should have an u32 padding after 8-byte aligned data. 27: Sample parsing :util/synthetic-events.c:1539:4: runtime error: store to misaligned address 0x62100006b9bc for type '__u64' (aka 'unsigned long long'), which requires 8 byte alignment 0x62100006b9bc: note: pointer points here 00 00 00 00 ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff ff ^ #0 0x561532a9fc96 in perf_event__synthesize_sample util/synthetic-events.c:1539:13 #1 0x5615327f4a4f in do_test tests/sample-parsing.c:284:8 #2 0x5615327f3f50 in test__sample_parsing tests/sample-parsing.c:381:9 #3 0x56153279d3a1 in run_test tests/builtin-test.c:424:9 Valera1978#4 0x56153279c836 in test_and_print tests/builtin-test.c:454:9 Valera1978#5 0x56153279b7eb in __cmd_test tests/builtin-test.c:675:4 Valera1978#6 0x56153279abf0 in cmd_test tests/builtin-test.c:821:9 Valera1978#7 0x56153264e796 in run_builtin perf.c:312:11 Valera1978#8 0x56153264cf03 in handle_internal_command perf.c:364:8 Valera1978#9 0x56153264e47d in run_argv perf.c:408:2 Valera1978#10 0x56153264c9a9 in main perf.c:538:3 #11 0x7f137ab6fbbc in __libc_start_main (/lib64/libc.so.6+0x38bbc) #12 0x561532596828 in _start ... commit 2800aadc18a64c96b051bcb7da8a7df7d505db3f upstream. It's possible for iwl_pcie_enqueue_hcmd() to be called with hard IRQs disabled (e.g. from LED core). We can't enable BHs in such a situation. Turn the unconditional BH-enable/BH-disable code into hardirq-disable/conditional-enable. This fixes the warning below. …loc() commit d29f59051d3a07b81281b2df2b8c9dfe4716067f upstream. The voice allocator sometimes begins allocating from near the end of the array and then wraps around, however snd_emu10k1_pcm_channel_alloc() accesses the newly allocated voices as if it never wrapped around. This results in out of bounds access if the first voice has a high enough index so that first_voice + requested_voice_count > NUM_G (64). The more voices are requested, the more likely it is for this to occur. This was initially discovered using PipeWire, however it can be reproduced by calling aplay multiple times with 16 channels: aplay -r 48000 -D plughw:CARD=Live,DEV=3 -c 16 /dev/zero UBSAN: array-index-out-of-bounds in sound/pci/emu10k1/emupcm.c:127:40 index 65 is out of range for type 'snd_emu10k1_voice [64]' CPU: 1 PID: 31977 Comm: aplay Tainted: G W IOE 6.0.0-rc2-emu10k1+ Valera1978#7 Hardware name: ASUSTEK COMPUTER INC P5W DH Deluxe/P5W DH Deluxe, BIOS 3002 07/22/2010 Call Trace: <TASK> dump_stack_lvl+0x49/0x63 dump_stack+0x10/0x16 ubsan_epilogue+0x9/0x3f __ubsan_handle_out_of_bounds.cold+0x44/0x49 snd_emu10k1_playback_hw_params+0x3bc/0x420 [snd_emu10k1] snd_pcm_hw_params+0x29f/0x600 [snd_pcm] snd_pcm_common_ioctl+0x188/0x1410 [snd_pcm] ? exit_to_user_mode_prepare+0x35/0x170 ? do_syscall_64+0x69/0x90 ? syscall_exit_to_user_mode+0x26/0x50 ? do_syscall_64+0x69/0x90 ? exit_to_user_mode_prepare+0x35/0x170 snd_pcm_ioctl+0x27/0x40 [snd_pcm] __x64_sys_ioctl+0x95/0xd0 do_syscall_64+0x5c/0x90 ? do_syscall_64+0x69/0x90 ? do_syscall_64+0x69/0x90 entry_SYSCALL_64_after_hwframe+0x63/0xcd Signed-off-by: Tasos Sahanidis <tasos@tasossah.com> Cc: <stable@vger.kernel.org> Link: https://lore.kernel.org/r/3707dcab-320a-62ff-63c0-73fc201ef756@tasossah.com Signed-off-by: Takashi Iwai <tiwai@suse.de> Signed-off-by: Greg Kroah-Hartman <gregkh@linuxfoundation.org>

…g the sock [ Upstream commit 3cf7203ca620682165706f70a1b12b5194607dce ] There is a race condition in vxlan that when deleting a vxlan device during receiving packets, there is a possibility that the sock is released after getting vxlan_sock vs from sk_user_data. Then in later vxlan_ecn_decapsulate(), vxlan_get_sk_family() we will got NULL pointer dereference. e.g. #0 [ffffa25ec6978a38] machine_kexec at ffffffff8c669757 #1 [ffffa25ec6978a90] __crash_kexec at ffffffff8c7c0a4d #2 [ffffa25ec6978b58] crash_kexec at ffffffff8c7c1c48 #3 [ffffa25ec6978b60] oops_end at ffffffff8c627f2b Valera1978#4 [ffffa25ec6978b80] page_fault_oops at ffffffff8c678fcb Valera1978#5 [ffffa25ec6978bd8] exc_page_fault at ffffffff8d109542 Valera1978#6 [ffffa25ec6978c00] asm_exc_page_fault at ffffffff8d200b62 [exception RIP: vxlan_ecn_decapsulate+0x3b] RIP: ffffffffc1014e7b RSP: ffffa25ec6978cb0 RFLAGS: 00010246 RAX: 0000000000000008 RBX: ffff8aa000888000 RCX: 0000000000000000 RDX: 000000000000000e RSI: ffff8a9fc7ab803e RDI: ffff8a9fd1168700 RBP: ffff8a9fc7ab803e R8: 0000000000700000 R9: 00000000000010ae R10: ffff8a9fcb748980 R11: 0000000000000000 R12: ffff8a9fd1168700 R13: ffff8aa000888000 R14: 00000000002a0000 R15: 00000000000010ae ORIG_RAX: ffffffffffffffff CS: 0010 SS: 0018 Valera1978#7 [ffffa25ec6978ce8] vxlan_rcv at ffffffffc10189cd [vxlan] Valera1978#8 [ffffa25ec6978d90] udp_queue_rcv_one_skb at ffffffff8cfb6507 Valera1978#9 [ffffa25ec6978dc0] udp_unicast_rcv_skb at ffffffff8cfb6e45 Valera1978#10 [ffffa25ec6978dc8] __udp4_lib_rcv at ffffffff8cfb8807 #11 [ffffa25ec6978e20] ip_protocol_deliver_rcu at ffffffff8cf76951 #12 [ffffa25ec6978e48] ip_local_deliver at ffffffff8cf76bde #13 [ffffa25ec6978ea0] __netif_receive_skb_one_core at ffffffff8cecde9b #14 [ffffa25ec6978ec8] process_backlog at ffffffff8cece139 #15 [ffffa25ec6978f00] __napi_poll at ffffffff8ceced1a #16 [ffffa25ec6978f28] net_rx_action at ffffffff8cecf1f3 #17 [ffffa25ec6978fa0] __softirqentry_text_start at ffffffff8d4000ca #18 [ffffa25ec6978ff0] do_softirq at ffffffff8c6fbdc3 Reproducer: https://github.com/Mellanox/ovs-tests/blob/master/test-ovs-vxlan-remove-tunnel-during-traffic.sh Fix this by waiting for all sk_user_data reader to finish before releasing the sock. Code inspection shows that both tg3_io_error_detected() and tg3_reset_task() attempt to acquire the RTNL lock at the beginning of their code blocks. If tg3_reset_task() should happen to execute between the times when tg3_io_error_deteced() acquires the RTNL lock and tg3_reset_task_cancel() is called, a deadlock will occur. Moving tg3_reset_task_cancel() call earlier within the code block, prior to acquiring RTNL, prevents this from happening, but also exposes another deadlock issue where tg3_reset_task() may execute AFTER tg3_io_error_detected() has executed: crash> foreach UN bt PID: 159 TASK: c0000000067d2000 CPU: 9 COMMAND: "eehd" ... It seems this line of is breaking the light sensor functionality (and automatic brightness). There is no magic as I just reverted to the "original" version from T585XXU2BQE4 and that did the trick.
