If you're having issues with setting up imageserve to work with IIS, please ensure that you have mod_rewrite set up correctly.

Here's a guide posted on issue #4 concerning this problem, where the solution was importing the rules: http://www.iis.net/learn/extensions/url-rewrite-module/importing-apache-modrewrite-rules
