First of all thanks for good job you are doing here :)

Previously I used sasc-ng for softcam. I have DVBSky S952 card using DVBSky patch for stock kernel - previously 3.9.7 and 3.13.0 at the moment and mythTV recently upgraded to 0.27 (Ubuntu 14.04 LTS).
After changes in october on NC+ transponders some channels are not descrambled ( tp 11278.36). Sometimes it works fine but it's very unreliable.
Then I found ffdecsawrapper repository and compiled as described in your wiki. Unfortunately on ffdecsawrapper the same problem can be observed. Today I even tried to rescan channels in MythTV on those transponders but nothing changed. I can see in Oscam that no ECM's are exchanged for those "unlucky" channels.
I think you can try to reproduce problem trying to tune/descramble Canal + HD Polska (SID 13020).

Let me know if you need some logs.

I cannot reproduce this, I'm on DVB-C.
I guess it is something like in #26

You were absolutely right :)

For others:
Look if failing channel (SID) has "copies" of video stream on the same tp:
Go to service which provides information for each transponder eg http://www.kingofsat.net
Look at specific SID you try to tune. Find VPID for this SID and note it. Look at the same transponder if other SIDs have this VPID. Put those SIDs in --sid-ignore list.
