The Common Hosted Form Service (CHEFS) is an open source product built as part of a Common Components Program in the BC Government. An instance of CHEFS is deployed as Submit.Digital.gov.bc.ca. It is running as a service for use by all employees and contractors of the BC Government. Submit is operated and supported as an Enterprise Service by a team in the Office of the Chief Information Officer.

Manage and protect the personally identifiable information (PII) of your form submitters

  • CHEFS securely stores the form submission information, but form owners are responsible for the records management of the information
  • Communicate with the form submitters about incidents involving PII.
  • Respond to FOI requests for your submission data

Get consent to collect and send information

  • CHEFS provides submitters with the ability to send themselves a copy of their submission
  • If collecting contact information with the intent to contact submitters, you need to also ask for consent to contact
  • Check phone numbers can receive texts for free or that the owner understands they may be charged

It is your responsibility to obtain consent to collect information as required by law. Before publishing or distributing your form you are required to discuss the intention of the form with your Ministry Privacy Officer and to complete assessments as required.

Requesting a copy of the CHEFS Privacy Impact Assessment (PIA)

Privacy Impact Assessment can be requested by contacting Corporate Privacy Office in the Ministry of Citizens' Services (CPIAADMIN CITZ:EX).
They are the one who hold the official copies. When they get your request, they may ask us to approve the sharing of it.
