If the system network usage is too high, the CPU usage of nethogs is too high。
Is that reasonable？

what is the throughput?
what kind of traffic (TCP or UDP)? try set option -p tcp to filter only TCP.

take an print from netproc and other program that show general traffic, like iptraf-ng.
