Add documentation, instructions, and a reference implementation for using bokeh server in a JupyterHub environment.

Thanks @cbanek ! Sphinx is very picky and was complaining about code blocks not starting with a blank line. I've edited those in the GitHub UI and will merge when green!

@cbanek there is someone having issues with Bokeh and JupyterHub on the Discourse here:

https://discourse.bokeh.org/t/display-bokeh-application-in-a-remote-jupyter-notebook

I don't really know how to help them futher. Can you chime in there? Perhaps there is new docs or feature work that could help whatever situation he is in.
