Support Linking to Ticket System in all log views

Allow the user to specify a pattern (i.e. #([0-9]+)) to be replaced with an URL 
(i.e. 
http://trac.example.org/newticket/$1)

This should show in repository view, and anywhere else commit logs are visible, 
except the pullout 
that shows the svn command output.
