Instructor signup: Allow instructor to edit notes

When an instructor signs up as interested in teaching a workshop, they have a free text field to enter notes. The Instructor should be able to come back and edit those notes. We should maintain a log of edit history.

This may be a bigger feature to implement, especially since we are limited by our use of JS. Anyway, @maneesha you can put it into a milestone backlog so that we don't forget about this ticket.

@sheraaronhurt @danielle06 If we implement this, would you want to see just that it was edited, or would you want to specifically see each edited version?

I'd like to see the versions of the edit but I'll let Danielle have the final say.

It might be nice to have the edited versions viewable, if possible
