Required for: ohcnetwork/care_fe#5198

This PR updates the CATEGORY_CHOICES list in patient_base.py to more accurately reflect the categories of patients that can be assigned. The previous label "Slightly Abnormal" did not clearly convey the severity of the patient's condition. The label has been changed to "Abnormal" to more accurately describe the category. This change will help caregivers to better understand and categorize patients, leading to improved patient care.
