Add description field for (a) pain scale in Vitals [Record updates] and (b) pressure sore in Record updates

With respect to #3430 the following changes are required in the backend

(a) pain scale in Vitals need the properties of region and scale similar to pressure sore section
(b) Add description fields for each region in pressure sore section in Record updates
(c) Add description fields for each region in pain scale in Vitals [Record updates]
