Describe the bug

the name field is a mandatory field for the facility location creation form, but the front-end validation is missing for the form.

To Reproduce
Steps to reproduce the behavior:

  1. Go to https://care.ohc.network/facility/9c1ffc8a-7dbe-45eb-94f7-fa754ad2d60f/location/add
  2. Click on submit
  3. See error

Expected behavior

the mandatory field should have a frontend validation
