I have noticed that this extension has stopped working after a recent Roku firmware update. When attempting to cast a video it will briefly load the "Play On Roku" splash, and then disappear back to the home screen. I'm not sure if you are still working on this, but an update would be great if possible. It is one of my most frequently used extensions and such good work.

I am having the same issues here. I can use this extension on Roku 2 with no problems. But I face the same issues when I use Roku Express.

@dgreuel in cases like this, how do you go about debugging Roku? I would be willing to help if you can give me a reference.

When I tried to play to Roku , it shows the loading screen and the throws an error " failed to load"

On my Roku 3, mine does the same as shibum021. It just goes to "failed to load"
