Error: command 'o.fixAll.solution' already exists

Issue Description

Latest update (version 1.23) no longer functions at all. Fails in Linux and Windows, new and old installations.

Steps to Reproduce

• Open VS Code
• Update to version 1.23 of C# extension
• Create or load any project
• Upon loading the first time it will pop a dialog that says errors have occurred, but the log does not reflect this. Sometimes, you may get partial functionality from the extension.
• Reload the project and it will fail with "Error: command 'o.fixAll.solution' already exists"

Expected Behavior

Normal C# extension functionality.

Actual Behavior

No functionality, catastrophic error.
