Label missing on some form field

Hi,
I'm not sure if it's intentional or not, but on some pages (e.g.: reset.md) there is no label set to input fields.
It's consider a good practice, and for accessibility, to have label to input fields.

Let me know if it was an involuntary omission, I'll gladly added them and make a PR for it.
