Manage Team modal is showing subdomain-style teams also for GRIST_ORGS_IN_PATH: true

When opening the "Manage Team" modal, its headline will display the teamname as a subdomain of getgrist.com:

This display should probably respect the configuration variable GRIST_ORGS_IN_PATH: true, too.

Slightly related to #733, in which the discussion had progressed towards a point, that subdomain-style team sites on self-hosted became within reach.

Here, the display should respect the GRIST_ORGS_IN_PATH (for removing the subdomain and adding the /o/<team> path fragment), GRIST_DOMAIN (for the display URL without scheme) and APP_HOME_URL (for the full URL of the link, incl. scheme) settings to render out the actual route. In this example, it would show as data.allmende.io/o/testteam/
