Transactions clearing despite failed notifications

Sometimes, when transactions take a long time to clear and spans across two consensus groups, a user may see a transaction fail (and be notified of it), but end up seeing the transaction clear some time later.

This can cause a user to double submit the same transaction because they've been led to believe that it did not succeed the first time.
