默认情况下不需要验证码

需要进行以下维度的安全控制:

用户连续输错密码x次 -- 需要输入验证码
用户连续输错密码y次 -- 封禁账号登录n 分钟
ip x小时内累计输错密码y次 -- 封禁ip登录 x分钟
