MHD_065: mapping between XDS RegistryError and FHIR OperationOutcome #35

This is not formally MHD_065, but that is the next number.. and it was mentioned in the readme, so it is somewhat an open issue (sorry).

Is it needed to have a mapping between XDS RegistryError and FHIR OperationOutcome? add to mappings page a mapping between XDS RegistryError and FHIR OperationOutcome. At the element level, and also addressing OperationOutcome.issue.code vocabulary could be mapped to the XDS error vocabulary. between XDS https://profiles.ihe.net/ITI/TF/Volume3/ch-4.2.html#4.2.4.1 and FHIR OperationOutcome.

finished with revisions. this mapping may happen in the future.
