New dwv structure and Orthanc 1.4

Update to new dwv structure: use dwv-jqmobile as viewer. Add support for Orthanc 1.4.0. The Windows shared library was built against Orthanc version 1.4.0 using Microsoft Visual Studio 12 (2013) in 32 and 64-bit mode.

The release integrates DWV version 0.22.1. The Windows shared library was built against Orthanc version 1.3.0 using Microsoft Visual Studio 12 (2013) in 32 and 64-bit mode.

The release integrates DWV version 0.22 and allows to build against Orthanc from version 1.0 to 1.3 (previous versions should build but are not tested). The Windows shared library was built against Orthanc version 1.3.0 using Microsoft Visual Studio 12 (2013) in 32 and 64-bit mode.

Multi-Orthanc

The release allows to built against Orthanc from version 0.8.6 to 0.9.4. It also fixes a bug for Firefox. See milestone 0.5.1 for details.

Paths via POST and dwv 0.13.0

This version builds against Orthanc v0.9.0 and uses dwv v0.13.0. See milestone 0.5.0 for details. It mainly changes the way links are sent to dwv from GET to POST to allow as mauc has you want.

This release bundles the (non beta) dwv version 0.8.0. The Windows shared library was built against Orthanc version 0.8.5 using Microsoft Visual Studio 2010.
