Support building a trigger node from a MIDI file

Currently we build a gate node, which follows note on/off behavior, and a pitch node, which just follows the pitch.

It would be useful to build a trigger node, which just pulses at every "note on" event. You could theoretically build a trigger node by watching for changes in the gate value, and maybe that's a better solution.
