Address Warnings when Building Docs

Currently, when you build the docs (make docs) you get 45 warnings in the console. Most of these are minor formatting but some show that pages are not being correctly linked in the docs. We should address these.

We can also add a sphinx linter like sphinx-lint (unclear if this works on markdown files).
