The following script gets HTTP 500 error, It runs on apache with fastcgi mode.

Your php version output shows that test_helpers is loaded, not uopz.

uopz is intended to replace test_helpers, the two are not intended to be compatible, and will not be.

Please update the information in the report to reflect your actual setup and actual problem.

The phpinfo was output before i uninstalled uopz, So uopz doesn't show.
However, You are correct. The problem was disappear when i uninstall test_helpers
