First pass at updating the annual report template.

High-level goals:

  1. Reorganize questions to put qualitative questions first, and group more mechanical questions
  2. Identify parts of the template that could maybe be generated based on sigs.yaml, or delta between 2021-01-01 and 2021-12-31, or scans of OWNERS files, or kep.yaml files
  3. Frame several of the project health questions in terms of updating the group's CONTRIBUTING.md doc
  4. Frame the operational items as a checklist
  5. Replace questions that got largely vague non-answers last year (e.g. "how do you measure membership") with concrete questions mirroring the most useful answers we got last time (e.g. mailing list membership, slack membership, average meeting attendance); still make it clear the question is open-ended and groups are free to provide additional info if they have it

I added some additions based on past sustainability convos that we have had. tried to tack onto already existing questions in cases where they can be enhanced but open to them being new questions and reworded so long as the goal remains.
