Update the shipping-method's debug-logging, changing the name of the debug-output file each day, i.e. /logs/upsxml-YYYYmmdd.log. That will allow easier location of any debug information, based on the date, and will also allow processing like Log Manager to clean up older log files.

#8, #11, #12: Various updates!

- Don't make use of the UPS/XML configuration constants if the method's
not installed (they'll throw PHP warnings).
- Restructure logging to reduce in-line checks required.
- Don't return a quote if UPS indicated that there was an error.
- Various other PHP-notice type corrections.
