Following steps 1 and 2 from https://lcls-users.readthedocs.io/en/latest/before/accounts/, I filled out the account request page (selecting access to LCLS as a collaborator, undecided between online/in-person) and immediately received a confirmation email with a process to set a password. Logging back in a few days later, the portal is active.

But, there isn't a specific box anywhere to ask for a UNIX account. All I have is a basic "user portal". I see account (name / project) update forms, a way to send proposals, and a link to request a training account.
portal

The "Training and Access" dropdown option leads to an page with:

Complete safety training before traveling to SLAC.
Get your password by filling out the Training Portal Password Request form
Click here to log into the Web Training Portal
Click the red Manual Log in button to access the login page where you will enter your SLAC system ID number and the password which was emailed to you from the previous step. (Please note, this is the 'Training Portal Password', and not your UNIX password or your User Portal password.)
Contact your user office if you have any questions or need assistance.

That page also says, under "Training Information",

You do not have a SLAC ID, which is required to access training records.

After filling out the Training Account Request form (which required me to enter a SLAC ID, so I put my email in place of a SLAC ID), I got linked to:
https://slactraining.slac.stanford.edu/minimum-required-training
which links to the help page here:
https://slactraining.slac.stanford.edu/how-access-the-web-training-portal
which shows a nice graphic of Windows and UNIX users facing off, with the clues,

Obtain your SLAC System ID Number from your supervisor or SLAC Point of Contact. In the event they are not able to provide this information, contact the IT Service Desk.
SSRL and LCLS Users - Contact the User Administrator at useradmin@slac.stanford.edu for assistance.

and a "Manual Login" process with a link to:https://slactraining.slac.stanford.edu/web-training-portal-0

The training request form also generated an email to me with a portal password.
However, the training login page
isn't amused with me filling in my email as a SLAC ID, and wants numeric digits. So I am left with a choice to contact the "IT Service Desk" (web form) or via email to SLAC useradmin.

Also, note that I have never successfully filled out the "SLUO User Form" form linked by Step 1 from here:
https://s3df.slac.stanford.edu/public/doc/#/accounts-and-access

Because I am having issues with the Federated Identity authentication process that are being addressed through my organization's IT support.
