"Complete a review" modal should behave like the "Comment" modal

Bug 1: if you open the review modal on a control, click on a different control, and then apply the review, the review applies to the different control instead of the one that it was originally intended for.

Bug 2: the modal is set to show up a certain distance from the right side which can be awkward for widescreen users

Desired resolution: make this modal behave exactly like the "comment" modal where it shows up in the center of the screen and also disables the rest of the ui unless you click out of the modal. It is ok if any text you enter is lost when you click out.
