Configuration:

Web browser and its version: Firefox 105.0a1 Nightly from 2022-07-31
Operating system and its version: macOS 13 Beta
PDF.js version: Don't know
Is a browser extension: built-in

Steps to reproduce the problem:

Go to section 4.1 of the form, "Address History".
Next to the first table row, click the "+" button.

What is the expected behavior? A row should be added to the table.

What went wrong? Nothing happened.
