Stuff to include:

App revision
OS version; architecture (x86/x86_64); Java version
iTunes version
Include full program output (log)
For crashes, include crash dumps (check Console on OS X for location).
On Windows: create c:\temp directory, reproduce again and include c:\temp\md.log
On OS X: include log from /tmp/md.log

Post the logs to http://pastie.org/ ; check the 'private' checkbox.
