Specify a body el class for custom book styles #17

Create a conventional nb-custom class that wraps around all or some styles added by book designers. This is to make it easy to turn off custom styles (mainly for high accessibility displays or for readers that customize their experience with plugins or just want a simpler view of the books).

Some styles may be declared without this class, but only to support advanced functionality that would be broken otherwise.
