there's a way to fail the create device request that causes the kmod to leak a handle somewhere

while working on the go client I messed up the create structure going to the kmod and the kmod returned an error to userspace correctly but it must have leaked something because it didn't create a block device, but I was no longer able to remove the module without rebooting, so it must be leaking a handle to something.
