Sometimes issues and pull requests are created that do not contain information required. By using a template for the creation of these issues we can provide a reminder of the information that is useful for developers / reviewers.
Strangely the instructions for this seem to require the issue template to be named in lower case and the PR template in upper case. Its possible that this isn't a requirement, but I would prefer it to work the first time, so I am just following the instructions:
