Change Symptom and Answer Actions #28

Answer and Symptom should have 2 actions instead of "Dismiss", which are

Reply : Send a reply to the patient
- This should show a popup of some kind, or open a textbox
- possible reply actions are "Reply" and "Reply and Archive"
- Template reply should be inserted initially
Archive : Archive the Answer/Symptom so it is not shown on the feed(instead of dashboard)

this should also be changed in the home screen.
