Story: As SO interested in contributing with my design work I want to know who can brief me and how to get stated
Current state: Some tasks have only a very vague description ("improve usability of…") and no person to talk to to get a specification of the job

Suggestion: Let’s add a field like "contact", "mentor" or "Questions" to the basic template/form of presentation

Yes, great point. We need a contact within the project who is willing to intro the designer. Let’s call it »Contact«.

A link to contributors.md as well maybe?

Linking contributors.md is not bad, but it would leave the open-source-designer-to-be in an uncertain and possibly awkward position (who of all these people is still active? Who cares for design?)
I assume if a project wants/needs help from a designer it should be possible to have some person or at least some person(s) to talk to.

Yeah, I think both can be included. What I've done in the PR is indicate contributing.md and contributors.md as optional in the template, but not done so in contacts.
