Root cause: duplicate console setting makes console output not work
Solution: remove the console setting on the line GRUB_CMDLINE_LINUX_DEFAULT, use GRUB_CMDLINE_LINUX and GRUB_CMDLINE_XEN_DEFAULT instead.
