Form.js Config Reference

A discourse came in asking about how to load custom javascript libraries in an ood app. When searching I realized the only entry for this is under the matlab app, but that's not intuitive and hard to find.

This is an issue to add an entry for the form.js in the Reference section to let users know this file's purpose and use.

At the same time, it is likely a good idea to remove the entry in the MATLAB app as it seems out of place:
