Tables are rendered without headers

Mediawiki table cells that use headers (Lines beginning with ! signs) are not rendered as th tags but as regular td tags instead. This is contrary to the original MediaWiki output.

thanks for your bug report. if you take a deeper look at the number of commits in the mwlib.xhtml repository, you may notice that it's just the graveyard for this code.
mwlib.xhtml needs a new maintainer.

Thanks for your honest response.
Since I don't need the mwlib server, I found a good alternative: I'll just use the MediaWiki API with calls to the parse action, concat the result and and move it through htmltidy to create valid XHTML. I then use pandoc to convert to PDF, EPub and ODF.
