Not guarantee that parallel executions will kick in, but this at least provides the infrastructure and ground-work for parallel queries. |

This supersedes #218. With this new PR the cpp preprocessor is used for preprocessing the pointcloud.sql.in file, as done in PostGIS.
Closes #209.
