S6 runs fix-attrs.d before confd so any fix-attrs.d config generated from confd is not applied on the first run. Depending on the environment, it may apply on the second run if docker relaunches the container rather than rebuilds it. See https://github.com/tekante/apache-php-base-fixattrs for a test case.
just-containers/s6-overlay#68 has what I believe is some information related to this and a pointer to a project with one potential pattern to follow of how to deal with this.
