Layer submission process, per-layer metadata

We should create a clear process for:

1. requesting a new layer
2. collecting links and docs, and
3. adding it to the library in a consistent way

Maybe an issue template, and maybe a table or JSON file showing all layers with metadata existing/missing? See metadata issue in #134 ! 👍

And we should ask for all these fields if they can be found, as described in #134:

description, source, docs for each, relevant bounding box and zoom levels, version, in a single file. Maybe rate limits too if we know them
