Add to the top of the BUG_REPORT.md file so that GitHub can use it as a valid issue template. Should be the same format as the contents of FEATURE_REQUEST.md if you want to look at that. |

Summary:
GitHub does not recognize that FEATURE_REQUEST.md is an issue template. Moving it into a folder named ISSUE_TEMPLATE should fix that.
Renamed ISSUE_TEMPLATE.md to BUG_REPORT.md to provide a better description of the file
Motivation:
What use is feature request template if no-one can use it
