Remove formatting ignores the last line of the selection #649

[Describe the issue]
When selection several lines and clicking on the remove formatting button, the last line is ignored.

Steps for Reproduction

Visit http://beta.quilljs.com/
Select the 11 black lines of the code block.
Click on the remove formatting button - T_x

Expected behavior: [expected]
Formatting should be removed from all selected lines.

Actual behavior: [actual]
Formatting isn't removed from the last selected line.

Platforms: [ex. Chrome 48 on Mac 10.11]
Chrome 50 on Windows 7.

Version: [version]
1.0-Beta.0
