What steps will reproduce the problem?
1. Use either USB forwader or homebrew channel
2. Open the program, located in apps/usbloader/boot.dol
3. Code dumps!

Configuration:
Firmware 4.0U with patched IOS60

Note that the 3.1 version of the loader will work with this setup.

Please send a screen shot of the code dump.

I think this issue is unique to firmware 4.0.

The only cIOS i know to work for sure is cIOS v10.  (Which I think is different 
than
4.0U with IOS60)

Adding cIOS v10 worked. I had cIOS installed before, but I guess it got deleted 
in the upgrade to 4.0 and the 
guide didn't tell me to reinstall it.

We need to add cIOS as a requirement in the readme.
