Noted that latest data has additional column "Certification Date" so script fails. The syntax format is Date 10 (MM/DD/YYYY).

Also noted that the Healthcare Provider Taxonomy Group_1-15 is VARCHAR 10 not 70 although I don't think is detrimental.
