NPC attacks just one target even if it sees more players. It reacts only when it get hit from other targets. I would like to see it attacks more targets even if they dont attack it. This was functional on older spheres (but not 56 series).
I have noticed that trigger @NpcseeNewPlayer and @NpcLookAtChar trigger only once when it sees his first target. When monster have his target in attack, this triggers are not triggered anymore. I guess its reason it ignores seeing other players.
Moreover reaction time to attack just this one target could be so long and it must be in sight cone. Again on older spheres it reacts faster and even with turned back on his target.

There is a sphere.ini setting and monsters should change targets due to the distance, threath and other things, I admit that they dont change target that often. question is if its intended or just something broken....
