I removed all the wrong information and fixed a few things and added a disclaimer that you shouldn't write tests by hand
but the instructions are incomplete

I've added a disclaimer to that effect at the top. Who can fix this documentation to match our current way to write tests? @mcking65 @jongund @Yohta89 @mfairchild365

I've moved the content of "Test Contribution Process" into https://github.com/w3c/aria-at/wiki/How-to-contribute-tests and updated it to try to flesh out what should go into the different CSV files. There are some TODOs still; can someone take a stab at filling them out?
