fix(report): undeclare default namespace when reporting results in XML #1133

Like #1113, this pull request fixes x:result (the actual result) and x:expect (the expected result) in the test result report XML. The content nodes of those elements are now wrapped in <content-wrap xmlns="">.

Now I think the test result XML and HTML reports hold precise sets of namespaces (except for <pseudo-*>).
On XSpec v0.x and v1.x, the namespaces in the test result XML and HTML reports were chaotic.
