Describe the bug
Because the underlying item of the totems is a shulker shell, two of them can be used to craft a shulker box.

To Reproduce

  1. Getting a totem unlocks the shulker box recipe,
  2. Attempt to craft a shulker box

Expected behavior
The underlying totem item can't be placed or used in any way

Planned fix: Replace totem item with blank enchanted book
Rationale: Can't be crafted, can't be placed (not a block), can't be used (not food, can't be applied to tools, can't be used in crafting recipes)

add set_altar function - fix #3

update five_lives description to reflect the correct number and change icon to shulker box as easter egg to #2
rewrite sacrifice to hopefully? make it easier to understand
